This facility to request documents is intended ONLY for students/alumni who attended:
- De La Salle University – Manila and
- De La Salle University – Laguna Campus, formerly De La Salle University-Canlubang / Science and Technology Complex
This facility does not cater to students/alumni from other De La Salle system schools, such as the College of St. Benilde, Medical and Health Sciences Institute, Dasmariñas, Araneta, etc.
For DLSU-Senior High School students, requests for documents may be logged via this link.
For transfer students, Form 137 (High School Transcript) and Form 138 (High School Card) should be requested from the school attended prior to DLSU.
The aforementioned transactions are INVALID and, thereby, cannot be processed.
