ID CARD CONCERNS (In-person Service)

The Enrollment Services Hub (ESH) handles the following transactions in connection with Identification Cards:

  • Replacement for Damaged ID cards
  • Replacement for Lost ID cards
  • ID Validation

ID Validation

  1. Schedule an appointment before proceeding to The Hub.
  2. Present appointment confirmation email.
  3. Get a queue ticket (ID matters).
  4. Hub Assistant will check and validate the ID card.

DRESS CODE FOR ID PICTURE-TAKING

  • Top with collar and sleeves (with or without coat);
  • No facial, ear, and neck jewelry;
  • No heavy makeup;
  • No eye glasses;
  • Hair pulled back to show full face and ears during ID picture taking; and
  • Clothing should be in contrast with a beige background.

Students

Manila-based

Replacement of Damaged ID cards

  1. Pay the Php597.00 ID fee:
    • at any DLSU-accredited payment channel or
    • at DLSU onsite cashiering counters. Secure payment referral slip from the Security Information Desk located at the North or South Gate.
  2. Proceed to the OUR – ID room for photo and signature capture. Students must conform with the dress code as stated above.
  3. Present Official Receipt and surrender the damaged ID card to the ID-in-charge.
  4. Secure ID claim stub containing a release date.
  5. On the release date, proceed to The Hub and get a queue ticket (ID Matters).
  6. Present the Official Receipt and the ID claim stub to the Hub Assistant.
  7. Claim ID card replacement.

Replacement of Lost ID cards

  1. Secure an Affidavit of Loss at your preferred Notary Public office.
  2. Proceed to the Security Information Desk located at the North or South Gate to report the lost ID card and present a Notarized Affidavit of Loss.
  3. Pay the Php597.00 ID fee:
    • at any DLSU-accredited payment channel or
    • at DLSU onsite cashiering counters. Secure payment referral slip from the Security Information Desk located at the North or South Gate.
  4. Proceed to the OUR – ID room for photo and signature capture. Students must conform with the dress code as stated above.
  5. Present Official Receipt and surrender the damaged ID card to the ID-in-charge.
  6. Secure ID claim stub containing a release date.

Laguna-based

Replacement of Damaged ID cards

  1. Pay the Php597.00 ID fee at any DLSU-accredited payment channel.
  2. Schedule an appointment before proceeding to The Hub.
  3. On the appointment date, be in your proper attire (see dress code) and present the following to the Hub Assistant for photo and signature capture.
    • Appointment confirmation email;
    • Official Receipt, and
    • Damaged ID card (to be surrendered)
  4. Secure ID claim stub containing a release date.
  5. On the release date, proceed to The Hub and present the Official Receipt and the ID claim stub to the Hub Assistant.
  6. Claim ID card replacement. 

Replacement of Lost ID cards

  1. Secure an Affidavit of Loss at your preferred Notary Public office.
  2. Proceed to the Security Information Desk located at the MRR Building or GT Building to report the lost ID card and present a Notarized Affidavit of Loss.
  3. Pay the Php597.00 ID fee at any DLSU-accredited payment channel.
  4. Schedule an appointment before proceeding to The Hub.
  5. On the appointment date, be in your proper attire (see dress code) and present the following to the Hub Assistant for photo and signature capture.
    • Appointment confirmation email; and
    • Official Receipt
  6. Secure ID claim stub containing a release date.
  7. On the release date, proceed to The Hub and present the Official Receipt and the ID claim stub to the Hub Assistant.
  8. Claim ID card replacement.

  1. Schedule an appointment before proceeding to The Hub.
  2. Present appointment confirmation email.
  3. Hub Assistant will check and validate the ID card.

Employees

Manila-based

  1. Secure ID referral slip from the following offices:
    • Office of the Provost (For Faculty and ASF)
