ID Card Releasing  –  UG Frosh ID 125

DLSU Student ID cards of Frosh 125 are released during the Lasallian Personal Effectiveness Program (LPEP), which is scheduled by college. An email containing the schedule will be sent to the students.

Steps to Claim ID card

  1. Ensure that you are logged in to your DLSU or your GOOGLE account.
  2. Check ID card availability by clicking the link corresponding to your college. You will be directed to a facility* that can be accessed via GOOGLE SHEETS.

    4. CLT-SOE

    7. RVRCOB
    8. SIS
    9. TDSOL

      *Note: The facility will be accessible starting August 15, 2025, Friday.        
  3. Present your Confirmation Slip or Enrollment Assessment Form (once available) at the service counter designated for ID releasing. 
  4. Access the ID log sheet. 
  5. Claim your ID card, ID lanyard, and ID holder.

FAQs

The Check ID facility gave a Not found result. What is the next step?

Non-availability of ID card may be due to the following reasons:

  • The UG Frosh 125 student submitted the confirmation requirements from July 28 to Aug 1, 2025. 
  • The ID card is still in production. 

Continue to access the Check ID availability facility. The system is updated daily for new ID cards that are produced. Once available, please refer to “Steps to claim your ID card” stated above.

I missed my ID picture-taking schedule during the Confirmation schedule. What should I do?

Proceed to the ID ROOM located at the Office of the University Registrar, 2nd Floor, Henry Sy Sr. Hall.

My ID card is available, but I was not able to claim it during LPEP?

Students can visit the Enrollment Services Hub, 2F Henry Sy Sr. Hall to claim their ID cards.

Can I send a representative to claim my ID card?

An AUTHORIZED REPRESENTATIVE can claim a student’s ID card. Please refer to the additional steps and reminders:

  1. Ensure to use the email address you provided in the DLSU Online Application facility.
  2. Access this GOOGLE FORM and upload the following:
    1. Signed authorization letter;
    2. Copy of the Enrollment Confirmation Slip or Enrollment Assessment Form (once available);
    3. Copy of the student’s valid ID and;
    4. Copy of the representative’s valid ID.
  3. The representative is to present his/her valid ID at the service counter. 
  4. ID cards will NOT BE RELEASED if requirements are INCOMPLETE.
