SUBJECT            :        Research Ethics Review Committee: Applications for Ethics Review for Term 3, AY 2023-2024

The DLSU Research Ethics Review Committee would like to inform the Lasallian community that to best manage the number of research ethics review applications, applications for review will be received two times a month, i.e. on the 1st and 3rd Friday of every month at 5:00 PM, except during work suspensions or breaks announced by the university. For Term 3 of AY 2023-2024, kindly take note of the following dates:

Proposals received will be classified according the type of review (Exempted, Expedited, or Full Review). The classification starts on the next working day after the indicated dates for those with complete submissions:

1. RERC Form 6A Application for Ethics Review
2. Final version of the full proposal
3. Informed Consent Form (ICF)
4. Assent Form (if applicable)
5. Data gathering instruments (questionnaires, surveys, FGD Questions)
6. Technical Review Certificate of Approval (from CRC/RGMO for those with internal funding; or for undergraduate/graduate students, from the thesis adviser and panel for accepting revised thesis proposal or dissertation proposal)
7. CV of proponents and co-proponents
