Non- Violence Contract (NVC) and Drug Testing Consent (DTC) Submission Guidelines (UG 123)
The University requires all incoming students to accomplish and submit a Non-Violence Contract (NVC) and a Drug Testing Consent (DTC) Form to ensure a safe learning environment for everyone.
Here are the guidelines for accomplishing and submitting the documents:

- Read and understand the contents of the contract.
- Fill-out ALL the needed information.
- Print all pages of the NVC.
- Have your declaration legally authenticated with a notary public.
- Scan or take a picture of the filled-out and notarized contract.
- Click here to access the Drug Test Consent.
- Read and understand the contents of the consent.
- Fill-out ALL the needed information.
- Sign the document together with your parent or your guardian.
- Scan or take a picture of the signed DTC.
- Submit the scanned copies or pictures of your NVC and DTC to this google form.
- The deadline for the online submission of the scanned copies or pictures will be on September 30, 2023.
- The hard copies of the NVC and DTC will be submitted during your CONFIRMATION DAY.
