Applying for Co-Academic Personnel’s (CAP) Web Access Guidelines

  1. The Department Chair shall make a requisition either by emailing the Help Desk or by filling-up a Service Requisition form (SRF). The request should contain the name/s of the CAP/s to be given an unlimited internet access.
  2. Upon receiving the request, Help Desk will make a Service Maintenance Request (SMR) for the Systems Administrator.
  3. The assigned Systems Administrator will set the CAP’s web access and return the accomplished SMR to the Help Desk.
  4. Help Desk will notify the Department Chair and the concerned CAP regarding the status of the request.
