Applications for Ethics Review for Term 3, AY 2024-2025

To better manage research ethics review applications, the DLSU Research Ethics Review Committee (RERC) will accept submissions every Friday until 5:00 PM. This schedule will be observed unless university-announced work suspensions or breaks are in effect. For Term 3 of Academic Year 2024-2025, please note the following specific dates:

The following are the required documents to submit for an ethics review application:

• Duly accomplished RERC Form 6A Application for Ethics Review
• Study proposal
• Informed Consent Form (ICF) (if applicable)
• Assent Form (if applicable)
• Data gathering instruments (questionnaires, surveys, FGD/KII interview guide)
• Technical Review Certificate of Approval (from CRC/RGMO for those with internal funding; or undergraduate/graduate students, from the thesis adviser and panel for accepting revised thesis proposal or dissertation proposal)
• CV of proponent, co-proponents, and thesis adviser

Note: For collaborations involving other institutions and countries, please include the mother proposal to provide a better context for the appreciation of the research proposal application.

Put all of the required documents into one (1) zipped folder and submit through the  Application for Ethics Review Submission Page.
