Research Ethics Review Committee: Applications for Ethics Review for Term 1, AY 2025-2026



The DLSU Research Ethics Review Committee would like to inform the Lasallian community that to best manage the number of research ethics review applications, applications for review will be received every Friday until 5:00 PM, except during work suspensions or breaks announced by the university. For Term 1 of AY 2025-2026, kindly take note of the following dates:

The following are the required documents to submit for an ethics review application:

  • Duly accomplished RERC Form 6A Application for Ethics Review (updated, as of Aug 2025)
  • Study proposal
  • Informed Consent Form (ICF) (if applicable)
  • Assent Form (if applicable)
  • Data gathering instruments (questionnaires, surveys, FGD/KII interview guide)
  • Technical Review Certificate of Approval (from CRC/RGMO for those with internal funding; or for undergraduate/graduate students, from the thesis adviser and panel for accepting revised thesis proposal or dissertation proposal or externally-funded projects)
  • CV of proponent, co-proponents, and thesis adviser

If not yet part of the proposal document, the following are other supplementary documents to submit for an ethics review application:

  • Recruitment Protocol
  • Debriefing Protocol
  • Marketing materials (ex: poster, social media ads)
  • Permit/s for restricted data and/or materials (ex:  permission from Provost, permission from private institution/s)
  • Permit/s for vulnerable populations (ex: NCIP clearance)

Additional Notes

  1. For collaborations involving other institutions and other countries, kindly include the mother proposal for better context appreciation of the research proposal application.
  2. For research that involves multiple phases (especially where results of one phase informs the direction/methodology of the next phase/s), this generally necessitates an iterative clearance.  In the proposal document, provide a title for each phase. There should be a separate document or section in the proposal document elaborating on the details of the phase that is being submitted for ethics review. The title of the phase and its relevant details are the ones that should be reflected in the RERC Form 6A.
