As part of the retirement age agreement which serves as an amendment to the 2021 Faculty Manual, all full-time faculty members are required to submit an annual performance scorecard. Please use the links below to download the Excel template:
The scorecards must be submitted to the Department/Unit, which will then be forwarded by the department/unit as a batch to the Office of the Provost through this link: Annual Performance Scorecard Submission on or before August 31, 2024.
