Applying for Co-Academic Personnel’s (CAP) Web Access Guidelines
- The Department Chair shall make a requisition either by emailing the Help Desk or by filling-up a Service Requisition form (SRF). The request should contain the name/s of the CAP/s to be given an unlimited internet access.
- Upon receiving the request, Help Desk will make a Service Maintenance Request (SMR) for the Systems Administrator.
- The assigned Systems Administrator will set the CAP’s web access and return the accomplished SMR to the Help Desk.
- Help Desk will notify the Department Chair and the concerned CAP regarding the status of the request.
