Personal information stored in the database of the school may be changed depending on certain circumstances.
The following are common cases where change of personal information may be done:
A change occurring or correction requested subsequent to the graduation or completion of program of the student at DLSU-Manila will not be entertained.
Graduates are provided a grace period of two (2) months from receipt of Diploma and souvenir copy of Transcript of Records to have errors corrected.
Documentary support
A request for change of personal information must be accompanied by certain documents, as follows:

*An affidavit may be executed only by a person of legal age and should be duly notarized. For an affidavit executed outside the Philippines, the same must be authenticated by the Philippine embassy/consulate in the country where the affidavit was executed.
Procedures
Change of personal information may be requested through the following steps:
1. Accomplish two (2) copies of the Request for Change of Information Form (Form No. R-50), available at the Front Desk of the Office of the University Registrar or downloadable here [http://www.dlsu.edu.ph/offices/registrar/pdf/R50.pdf]
2. Submit the accomplished Request for Change of Information Form, together with the required documentary support, to the Front Desk of the Office of the University Registrar.
A request for change of personal information is decided and will take effect after four (4) working days from submission of documents.
