Transfer Application Procedures


Criteria


  1. Grade Point Average (GPA) of at least 85% or its equivalent
  2. No failure in any subject
  3. No incomplete grades
  4. No unofficial dropped subjects

Procedure


  1. Completely fill out the Pre-Application Form (Downloadable at http://www.dlsu.edu.ph/admissions/undergraduate/forms/admission-pre-application-form.pdf) and submit personally or through mail/email together with the following:

1.1 Transcript of Records (TOR) with grading system or copy of grades issued by the Registrar.

(Note: If enrolled in a trimestral system, midterm grades are required to be submitted)


  1. Upon evaluation of the Pre-Application form and the Transcript of Records, the Office of Admissions and Scholarships will notify if the student has qualified to take the exam. If qualified, submit the following:
  2. 2.1 Online Undergraduate application Form (http://www.dlsu.edu.ph/admissions/undergraduate/applicationformnew/Introduction.aspx)


    2.2 Two (2) recommendations (Use the DLSU Recommendation Form) from any two of the following:


    • Dean
    • Department Head
    • Professor
    • Guidance Counselor

    2.3 Three (3) identical passport photographs

    2.4 Photocopy of the Birth Certificate (NSO for Filipino citizens)

    2.5 Certificate of Good Moral Character

  1. Pay the non-refundable Application Fee of PhP600.00 (Filipino Citizens) or $50.00 (non-Filipino Citizens).
  2. Present the Official Receipt to the Office of Admissions and Scholarships and secure Admissions Kit and Test Permit.
  3. Take the exam on the scheduled date. All applicants must present their exam permits on the date of examination.

    3.3 Three (3) identical passport photographs
    3.4 Photocopy of the Birth Certificate (NSO for Filipino citizens)
    3.5 Certificate of Good Moral Character

  1. Pay the non-refundable Admission Processing Fee of PhP600.00 (Filipino Citizens) or $50.00 (non-Filipino Citizens).
  2. Present the Official Receipt to the Office of Admissions and Scholarships and secure Admissions Kit and Test Permit.
  3. Take the exam on the scheduled date. All applicants must present their exam permits on the date of examination.

Note:
Application to the preferred degree program is subject to the availability of slot per term. The Office of Admissions and Scholarships reserves the right to deny admission to any applicant who submitted all admission requirements but has incurred failing and/or incomplete grade/s, unofficially dropped subjects and obtained a grade point average lower than 85% or its equivalent as indicated in the final copy of the Transcript of Records (TOR).
