Submission of Panel Report for Term 2, AY 2025-26 (Graduate Studies)

The Office of the University Registrar would like to remind all concerned departments that the deadline for submitting the Non-Coursework Graduate Studies Panel Report Grades for Term 2, AY 2025-26 is on 17 April 2026, Friday until 8:00 PM.

The process for the grade submission for non-coursework will remain as is for this Term 2, 2025-26.

After all the required approvals/signatures are completed, the PDF copy of the Panel Report needs to be submitted to the Office of the University Registrar by an authorized person in the department (Secretary, Panel Chair, Department Chair, Office Manager, and Graduate Program Coordinator) through the following Google form:

Note: 

  • Any forms submitted by the students will not be accepted.
  • Approval. The Panel Report forms should undergo the usual round of signatures indicated on the form. We will accept the forms with an e-signature.
  • Submission of Grades beyond the deadline (17 April 2026) will no longer be accommodated.
  • For graduating students for Term 2, 2025-26, ifthe final defense panel requires revisions in the thesis/dissertation, the concerned department should submit the revised version (Panel Chair Report) on 12 May 2026.
  • All Panel Chair Reports submitted beyond 12 May 2026 will not be considered for processing for inclusion in the Final Graduating List for Term 2, 2025-26. Students will have to apply to graduate for the next term.
  • Students completing revisions must enroll in Thesis Writing if mentoring is necessary; otherwise, residency enrollment is required until the modification is approved.
  • Students are encouraged to ensure the timely submission of forms to OUR by their respective Departments to avoid inconveniences.
