Requirements

These forms will help the Scholarship and Financial Assistance (SFA) Committee evaluate your need for financial assistance while studying at De La Salle University. It is very important that you complete them honestly and accurately.

There are two (2) forms. These are the Personal and Educational Background Form (pdf/586kb) and the Recommendation for Financial Assistance Form (pdf/548kb).

The following documents must be submitted together with these forms:

1. A photocopy of your parents’ Income Tax Return documents for the previous year
2. A letter of request for financial assistance from the applicant, addressed to the SFA Committee, stating the following:
   • reason(s) for applying for financial assistance
   • reason(s) for choosing De La Salle University-Manila
   • other circumstances which the applicant believes to have a bearing on the application for financial assistance

All documents and data that you will provide shall be held in strict confidence and will be used solely for evaluation purposes.

Your application for financial assistance will not be processed unless all the required information and documents have been provided.
