Issuance of Transcript of Records

The Transcript of Records is an inventory of the courses taken and grades earned of a student throughout his stay in De La Salle University, including transferred credits from other schools.

The Transcript of Records may be official (with school seal and signature of the University Registrar) or unofficial.

The Transcript of Records may be issued as requested, regardless of frequency and number of copies.

Processing Period

The Transcript of Records may be processed as Regular or as Express.

Processing Period
Regular Processing The document is available after four (4) working days from the date of submission of request
Express Processing The document is available on the next working day from the date of submission of request.
Cut-off for express processing shall be 12 noon. Requests submitted after the cut-off time shall be deemed submitted the following working day.

Note:Saturday is not counted as a working day for determining due date of request.

 

Procedures for Requesting

Copies of the Transcript of Records may be obtained through the following steps:

  1. Request
  2. Assessment
  3. Payment
  4. Delivery

The Transcript of Records may be requested on-line or manually at the Front Desk of the Office of the University Registrar.

For on-line requests, assessment is done electronically, payment via credit card, and delivery via courier. Please indicate any special instruction in the comment/message box.

Manual Request
To request for the Transcript of Records manually, please visit the Front Desk of the Office of the University Registrar and follow the procedures below:
  1. Download the form REQUEST FOR DOCUMENTS (Form No. C01) or secure it at the Front Desk of the Office of the University Registrar.
  2. Fill out the form and submit it to the Front Desk of the Office of the University Registrar for assessment. (See Schedule of Fees)

    NOTES: The requesting party may opt to download the form and have it assessed by the Office of the University Registrar. Please advise our staff at the Front Desk for any special instruction.

  3. Pay the assessed amount at the Accounting Office and have the form machine validated.
  4. Return the form to the Front Desk of the Office of the University Registrar and present the Official Receipt.
  5. On the scheduled date of release indicated in the Official Receipt, claim the Transcript of Records at the Front Desk of the Office of the University Registrar.

NOTES:

To claim the Transcript of Records, the following options are available:

The Transcript of Records may be picked up personally. The DLSU ID (student) or any valid ID (alumnus) and the Official Receipt for the payment of the Transcript of Records need to be presented.

The Transcript of Records may be picked up by a proxy/representative. A duly-accomplished and signed PROXY REQUEST OF RECORDS (Form C-06) needs to be presented.


Courier Services

The Transcript of Records may be sent via courier to a designated address.
(See Delivery Schedule and Rates).

The delivery period is over and above the processing period. Transcript of Records not claimed after sixty (60) days from the date of request will be discarded.
