Issuance of Certifications


The Office of the University Registrar issues various types of Certifications. These include Certification of:

  1. Enrollment
  2. Units Earned
  3. Candidacy for Graduation/Academic Completion
  4. Graduation/Graduation with Honors
  5. Cumulative Grade Point Average (GPA)
  6. Dean's List
  7. Grades
  8. English as medium of instruction
  9. Ranking (for undergraduate students only)
  10. Others (only for matters with readily-available data and are verifiable)

Certifications may be issued as requested, regardless of frequency and number of copies.

Processing period

The Certifications may be processed as Regular or as Express.

Processing Period
Regular Processing The document is available after four (4) working days from the date of submission of request
Express Processing The document is available on the next working day from the date of submission of request.
Cut-off for express processing shall be 12 noon. Requests submitted after the cut-off time shall be deemed submitted the following working day.

Note:Saturday is not counted as a working day for determining due date of request.


Procedures for requesting

Copies of the Certifications may be obtained through the following steps:

  1. Request
  2. Assessment
  3. Payment
  4. Delivery

The Certifications may be requested on-line or manually at the Enrollment Services Hub (ESH).

Request for Documents
Online Request

Request for Certifications online.

For online requests, assessment is done electronically, payment via credit card, and delivery via courier. Please indicate any special instruction in the comment/message box.

Manual Request

To request for the Certifications manually, please visit the ESH and follow the procedures below:

  1. Download the form Request for Documents (Form No. C-01) or secure it at the Enrollment Services Hub (ESH).
  2. Fill out the form and submit it to the ESH Staff.

    NOTES: The requesting party may opt to download the form and have it assessed by the Enrollment Services Hub (ESH). Please advise our staff at the ESH for any special instruction.

  3. Pay the assessed amount to the Cashier and have the form machine validated.
  4. Return the form to the ESH Staff and present the Official Receipt.
  5. On the scheduled date of release indicated in the Official Receipt, claim the Certifications at Enrollment Services Hub (ESH).

NOTES:

To claim the Certifications, the following options are available:

The Certifications may be picked up personally. The DLSU ID (student) or any valid ID (alumnus) and the Official Receipt for the payment of the Certifications need to be presented.

The Certifications may be picked up by a proxy/representative. A duly-accomplished and signed Proxy Request of Records (Form C-06) needs to be presented.

The Certifications may be sent via courier to a designated address. See Delivery Schedule and Rates. The delivery period is over and above the processing period. Certifications not claimed after sixty (60) days from the date of request will be discarded.
