SSS Number Registration

On Applying for an SSS Number Requirements 
Submit any of the following documents:

• BIRTH CERTIFICATE (Certified True Copy)
• BAPTISMAL CERTIFICATE (in the absence of birth certificate)
• PASSPORT

In the Absence of the above documents, submit any TWO (2) of the following:

• Record of Employment / ER ID
• Marriage Contract
• Birth / Baptismal
• Certificate of Children
• School / Voter's Identification Card
• Driver's License
• GSIS Member's Record
• Certificate from the National Archive
• Alien Certificate of Registration
