Student's Annual Physical Examination 2009

ALL UNDERGRADUATE STUDENTS enrolling for SY 2009-2010 are REQUIRED to undergo the Annual Physical Examination (Periodic Health Examination) to be conducted by the University Clinic from January 12 to March 14, 2009.

A.P.E. Procedures

1. On your assigned schedule (see adjacent poster), have your:
   • PHYSICAL EXAMINATION at the Waldo Perfecto Seminar Room (SPS ground floor)
   • CHEST X-RAY taken at the X-ray Van parked between the back of Animo Canteen and SPS Building
2. Get your chest x-ray result from the Clinic seven (7) school days after your physical examination.
3. A Clearance slip will be issued by the Clinic after you have completed steps 1 and 2.
   The Clearance slip is needed in order to get your EAF for the First Trimester, 2009-2010.

Note: Presentation of your valid school ID is required for procedures 1 and 2.

Schedule

• 8:00 a.m. to 4:30 p.m. Monday to Friday
  NO LUNCH BREAK
• 8:00 a.m. to 11:30 a.m. Saturdays

Reminders:

• If you have a problem with your schedule, please inform the Clinic ahead of time.
• Please bring a ball pen for filling out your APE form.
